Just speaking from own similar troubles: It could be a nerve impengement
where the nerve to your shoulder exits your cervical spine, or an
impingement in the shoulder itself. A good chiropractor can evaluate these,
and either treat them or refer you to an MD. Also, simply sleeping with
your head or shoulder in a stressed position can leave a kink that stays
after you get up. Movement can be helpful: I get benefits from yoga or
other workout most mornings.

 An MRI can reveal structural/functional problems that even the best chiropractor or physical therapist cannot. Assuming that something like shoulder cuff surgery is not indicated, diagnostic imaging can suggest what therapies, manipulations or exercises would help and which might cause further damage. 

FWIW, although we have lots of experience with chiropractic, yoga, acupuncture, and physical therapy, my wife had the surgery, and it was successful. I got evaluated and found that my condition didn't warrant the surgery (which I didn't want anyway) but that some exercises I was doing were making it worse.

Peter,

When I had a shoulder problem, my personal trainier correctly identified
the cause as an impinged supraspinatus. She gave me some specific exercises
with light weights and a resistance band that sorted out the problem. I
don't know whether you have the same underlying problem or whether the same
fix might work for you, but I'd say that for this sort of thing, consulting
a physical therapist or a personal trainer with rehab expertise could be a
good way to zero in on the cause and eliminate it.
